Your Direct Dental ID Card is now Digital!

Looking for your ID Card? You don’t need an ID Card to get dental care

Simply tell your dental office that you’re covered by Direct Dental and provide your name, date of birth, social security number and the name of your employer. The office will use that information to contact our Provider Services team at 855-866-2615 to verify your eligibility and benefits. If you have dependents on your plan, tell them to provide your details.​​

But what if I want an ID Card?

If you would still like an ID Card you may access your digital card online:

• Log in to the Member Web Portal

• Click on Enrollment, then Member ID Card

• Take a photo of your ID Card and save on your smartphone, or print your ID Card and bring to your dentist.
