Your Direct Dental plan may include coverage for specified vision services

Not all Direct Dental plans cover vision services. Your Plan Benefit Summary describes the services covered by your plan.  Your Plan Benefit Summary is available to you in the Member Web Portal or from your HR team.

Have vision coverage? Access your vision benefits in 4 easy steps:
  1. If your plan provides coverage for vision services, you may see any vision provider you wish.

  2. You will need to pay out-of-pocket for vision services upfront and obtain an itemized receipt for your expenses at the time of service.

  3. Submit a Vision Claim Reimbursement Form along with a copy of your itemized receipt.

  4. Reimbursement will be sent via check within 30 days provided your claim is eligible and complete.
